All rental and purchase orders are taken via telephone.

You may place your order by calling, toll free 800-375-6060, between 8:00 AM and 5:00 PM Mountain time, Monday through Friday.

All rentals and purchases require a medical prescription signed by a doctor. Airline authorizations signed by the doctor are acceptable for an oxygen prescription. These may be faxed directly to Liberty Medical at 303-279-8706. The doctor’s order must be less than one year old.

Information your Liberty Medical representative will need to know:

• Your name, address, and telephone number.
• The type of system you wish to rent or purchase.
• The number of batteries you will need.
• Travel dates, start date and end date (for rented equipment).
• Your credit card information, Visa and Mastercard accepted.
